The player is responsible to email (joanne@enysoccer.com  ), fax (516-678-7411) or mail (ENYYSA, 265 Sunrise Hwy., Suite 38, Rockville Centre, NY 11570) a copy of birth certificate.  A passport size photo must be uploaded when registering.

Upon verification, the State Office will email an "Approved" receipt, providing permission to play out of state.

 Team Registration

Upon registration, the state will obtain your League's approval.  The State Office will email an "Approved" receipt.
